โรคพยาธิไส้เดือนเป็นโรคที่เกิดจากการติดเชื้อพยาธิไส้เดือน (Ascaris lumbricoides) ซึ่งเป็นพยาธิตัวกลมชนิดหนึ่ง โดยการกินไข่ที่มีตัวอ่อนของพยาธิไส้เดือน* ระยะติดต่อเข้าไปอยูในร่างกาย ทำให้เกิดอาการต่าง ๆ ตามอวัยวะที่ตัวพยาธิเข้าไปอาศัยอยู่
โรคนี้พบได้ในคนทุกวัย แต่จะพบในเด็กมากกว่าผู้ใหญ่ และในเด็กมักมีอาการรุนแรงกว่า เนื่องจากมักจะมีพยาธิในลำไส้เป็นจำนวนมาก
โรคนี้พบมากทางภาคใต้ และอาจพบในภาคอื่น ๆ ได้พอประมาณ
*วงจรชีวิตของพยาธิไส้เดือน
1. พยาธิไส้เดือนตัวเต็มวัย (ตัวแก่) มีรูปร่างคล้ายไส้เดือนสีขาวยาวประมาณ 20-40 ซม. อาศัยอยู่ในลำไส้เล็กของคน ตัวเมียจะออกไข่วันละนับแสนฟอง ซึ่งจะออกมากับอุจจาระ หากคนถ่ายอุจจาระลงพื้นดินหรือในน้ำ ไข่จะอยู่ตามสิ่งแวดล้อม (น้ำ ดิน ทราย ฝุ่นละออง ผักที่ใส่ปุ๋ยทำจากอุจจาระคน)
2. ไข่ที่ไม่ได้รับการผสม แม้คนกินเข้าไปก็ไม่ทำให้เกิดการติดเชื้อ
3. ส่วนไข่ที่ได้รับการผสม จะเจริญเป็นตัวอ่อน (ในไข่) ในเวลา 10-21 วันซึ่งเป็นระยะติดต่อ
4. เมื่อคนกินอาหาร ผัก ผลไม้ น้ำดื่ม หรืออมนิ้วมือที่เปื้อนไข่พยาธิดังกล่าว ก็จะกลืนเอาไข่พยาธิที่มีตัวอ่อนเข้าไปในลำไส้
5. ตัวอ่อนในไข่จะฟักออกมาเกาะอาศัยอยู่ตามลำไส้
6. ตัวอ่อนจะไชทะลุเยื่อเมือกของลำไส้เข้าไปตามระบบไหลเวียนเลือดและระบบน้ำเหลืองเข้าสู่ปอด และเจริญต่อไปภายในปอดซึ่งใช้เวลา 10-14 วัน แล้วไชทะลุผ่านผนังถุงลม เคลื่อนตัวขึ้นไปตามหลอดลมจนถึงคอหอย
7. ตัวอ่อนจะถูกกลืนกลับลงไปในหลอดอาหารและลำไส้อีกครั้งหนึ่ง คราวนี้ตัวอ่อนก็จะเจริญเป็นตัวเต็มวัย และอาศัยอยู่ในลำไส้ ซึ่งสามารถมีชีวิตได้นานถึง 1-2 ปี
สาเหตุ
การติดต่อของโรคนี้ เกิดจากการกลืนไข่พยาธิไส้เดือนระยะติดต่อที่ปนเปื้อนอาหาร ผัก ผลไม้ น้ำดื่ม หรือนิ้วมือ
อาการ
ถ้ามีพยาธิอยู่ในลำไส้จำนวนน้อย มักจะไม่มีอาการอะไร บางรายอาจถ่ายหรืออาเจียนเป็นตัวไส้เดือน
บางรายอาจมีอาการปวดท้องหรืออาเจียนเป็น ๆ หาย ๆ เรื้อรัง โดยมักจะมีอาการหลังกินอาหารสัก 1/2 ชั่วโมง
บางรายอาจแสดงอาการลมพิษเรื้อรัง
ในรายที่มีพยาธิจำนวนมาก เด็กอาจมีอาการผอมแห้งแรงน้อย กินข้าวได้แต่ไม่อ้วนขึ้นหรือกลับผอมลง บางรายอาจมีอาการเบื่ออาหาร ปวดท้อง คลื่นไส้ ท้องเดิน
บางรายมีลักษณะพุงโรก้นปอด ขาดอาหาร
ภาวะแทรกซ้อน
ในเด็กอาจเกิดภาวะขาดอาหาร (เช่น ขาดสารโปรตีน วิตามินเอ) เนื่องจากพยาธิไส้เดือนแย่งอาหารในลำไส้
บางครั้งพยาธิอาจรวมเป็นกระจุก ทำให้เกิดอาการอุดกั้นของลำไส้ มีอาการปวดท้องรุนแรง อาเจียนรุนแรง และคลำได้ก้อนที่หน้าท้อง
พยาธิตัวอ่อนที่เคลื่อนตัวผ่านปอด ถ้ามีจำนวนมากอาจทำให้เกิดอาการไอหรือปอดอักเสบ
บางครั้งพยาธิตัวแก่อาจเคลื่อนตัวเข้าไปอุดกั้นในท่อน้ำดี ทำให้มีอาการดีซ่าน และถุงน้ำดีอักเสบ, เข้าไปในอุดกั้นในท่อตับอ่อนทำให้ตับอ่อนอักเสบ, เข้าไปในตับทำให้เป็นฝีตับ, เข้าไปในรูของไส้ติ่ง ทำให้ไส้ติ่งอักเสบ
ภาวะแทรกซ้อนที่ร้ายแรงแต่พบได้น้อยมาก ได้แก่ เป็นแผลที่ลำไส้และมีเลือดออกรุนแรง ลำไส้ทะลุ ลำไส้เน่า ซึ่งอาจทำให้เสียชีวิตได้
